Community Connection Project (CCP) program is a nonprofit, 501©3 transitional, post high school (12th grade) program for young adults with developmental disabilities such as autism spectrum disorder. The CCP program is designed to teach community skills, seek out community resources and adult support networks, and provide opportunities for young adults with social and behavioral challenges to connect with their community. Community Connection Project approaches its vision and purpose through three phases – all of which are necessary for successful inclusion, meaningful contributions, and high quality of life for participants and their community members.

The Community Connection Project Vision
The vision of Community Connection Project is to build the bridge between school and adulthood by providing specialized training and support for young adults with developmental disabilities and their chosen adult-life networks. We provide our program participants opportunities to identify, train, and build upon natural adult support networks, foster meaningful relationships, and generalize the life skills needed to connect with their community successfully and inclusively.

Program Main Informational Summary Points

  • Community Connection Project is a unique three-phase program, bridging high school to adult life connections and support for program participants with developmental disabilities such as autism spectrum disorder.

    • We specialize in working with individuals with more significant social, communication and behavioral challenges that require a very specialized level of support.

  • CCP works with program participants from 18 years old onward who are no longer in high school but may be in a school-based transition program.


Hours Of Service

  • CCP is meant to be a “Bridge the Transition Gap” program for participants that have exited their high school programs within the prior 6 years and need specialized training options for their chosen networks to help ensure success and an enhanced quality of life.

  • CCP will provide collaborative and experiential opportunities to identify, transition, and train all chosen, social, and support community relationships, networks, and services.

  • CCP is community-based, meeting at a designated drop off and pick up spot (which may change based on individual circumstances) for transportation to and from the program.

  • Program participants must be ambulatory and able to be transported safely with others, toilet trained, and able to safely proceed into the community with at least one-to-one or two-to-one staff support.

  • Family, caregiver, and community network training will be offered in each individual’s program and transition plan.

  • CCP will incorporate participant-run micro business(es) to provide real-world training and vocational experience to our program participants within the first year of operation (timing on start dates for this component is TBD)

    • Tuesday - Friday

      • 18 - 22+ yrs old: 2:00 PM - 6:00 PM with some flexibility on both sides

      • 22+ yrs old: 12:00 PM - 5/5:30 PM with some flexibility on both sides

    • Saturday

      • 22+ yrs old: 9:00 am to 12:30 PM

      • 18 - 22+ yrs old: 1:00 to 4:30 PM


* Days of attendance will be determined by the person centered planning and transition team and may change as the program grows, peer groups change, and expansion becomes necessary.

Program Outcome Goal
By the time program participants exit CCP, they will be in their permanent chosen placements including but not limited to vocational/volunteer, living arrangement, social, recreational, and other adult service choices with a network of TRAINED service providers, natural community supports, and relationships. They will enter into their adult world with the greatest chosen prospects for a fulfilled, happy, successful life as possible.

Our main focus will ALWAYS be on being an example of how important it is to have a culture of all-ability inclusion in our community!
